(10182842) CA Dept of Water Resources: w/ *ADDENDA 1-3* Industrial Hygiene (Northern California). The Contractor agrees to provide to the Department of Water Resources (DWR), Division of Flood Management (DFM), as needed industrial hygiene services. including cleaning, disinfecting, sanitizing, safety support (project oversite, inspections, incident investigations, training, etc.), quality control testing, removal and disposal, abatement of hazardous or non-hazardous spills, quality control sampling and quality control oversight services to the DWR, DFM facilities, jobsites, and equipment. Contractor will be licensed in accordance with the laws of the State of California and will provide proof of the following: 1. Strike Team a. Must possess Valid Class A Contractors License with Hazardous Substance Removal Certification b. Must possess Medical Waste Transporter License 2. Hazardous Waste Handling a. Valid General Engineering Contractors Class A b. Hazardous Substance Removal Certificate issued by the California Department of Consumer Affairs, Contractors State License Board c. Hazardous Waste Transporter Registration from Department of Transportation (DOT) d. Motor Carrier Permit issued by the State of California, Department of Motor Vehicles (DMV) e. Hazardous Materials Transportation License issued by the California Highway Patrol (CHP) f. California Integrated Waste Management Board Waste Tire Hauler Permit g. Current U.S. DOT Registration Number and an EPA identification number h. Contractor must further provide qualified drivers who have an endorsement to haul hazardous waste on their operator's license from the Department of Motor Vehicles (OMV). 3. All Personnel must be current with all required training prior to assigned duties with the DIR's, Cal/OSHA, Title 8 Regulations, 49 CFR's 171, 49 CFR's 172 and 22 CCR Division 4.5 Sections 66260 to 69100 and when required, is properly certified or licensed in accordance with federal, state, or local laws and regulations. In addition, Contractor must provide proof personnel have completed the Hazardous Waste Operations and Emergency Response HAZWOPER training requirements for site clean-up operations as specified in Title 8 CCR §5192 (e)(3) and for emergency response operations as specified in Title 8 CCR§ 5192 (q)(6)(c). 4. Safety Support – Contractor must provide list of qualified personnel and proof of qualification for each of the following individuals: a. Certified Industrial Hygienist (CIH) b. Certified Indoor Environmental Consultant (CIEC) c. Certified Asbestos Consultant (CAC) d. Certified Site Surveillance Technician (CSST) e. Certified Microbial Consultant (CMC) f. Certified Microbial Inspector (CMI) g. California Department of Public Health (CDPH) Lead-Related Construction (LRC) certified h. Certified Lead Inspector (CLI) i. Registered Environmental Health Specialist (REHS)
